Assessments:
All clients must be assessed to determine if they meet the definition of “Independent” for a Room and Board.
Assessments are video taped to document a client’s status of ambulatory or ability to self-transfer from wheelchair to bed and bathroom(if applicable).
Client will be asked about any issues that impact housing location(I.e., drugs, alcohol, criminal background, any PTSD issues that need to be considered). *Note: these do NOT disqualify someone for housing but is necessary information to get the best fit for client and R&B operator
Clients will be asked about Budget, location, type of room, and meal request.
A Coalition member does the first assessment and shares the information with the members to find availability and best fit. The client will then be contacted by the member(s) and will be able to select from who has availability.
Placement:
One client and operator have come to an agreement, the coordinating caseworker is contacted and provided with the details of the agreement.
The Caseworker coordinates discharge or move-in date.
The Caseworker coordinates home health, PT, OT, and any DME necessary. (NOTE: any DME must be discussed with the operator prior to ordering and sending equipment to ensure it is proper for their home.)
Transportation:
The client is to transport themselves
Hospital provides transportation.
